Community Healthcare Network is seeking for a Full-Time RN Care Coordinator/Buprenorphrine Services who will be responsible for setting up and organizing buprenorphine program, identifying providers to be trained and wavered, identifying the site needs for all service locations. Will also recruit, screen and assess patients, coordinate and manage care of patients being treated and champion buprenorphine prescribing within the practice. The RN will also be responsible for coordinating and supervising team members providing services, team meetings, patient recruitment, and retention of patients, close follow-up, and making outside referrals as needed. The RN will also be responsible for ensuring program follows state and federal guidelines and meets implementation timeline of the grant.

DUTIES AND RESPONSIBILITIES:

1. Works with Deputy Director of Psychiatry and Vice President of Behavioral Health to design workflow of program.

2. Collaborates with development department and Grant funder to assure proper structure and reporting in place.

3. Conducts trainings for clinical and operational staff to facilitate program, patients' needs, and workflow.

4. Outreaches to patients throughout network as well as providers to engage them in care with program.

5. Outreaches to external facilities to design and receive external patients as needed.

6. Collaborates with Social Worker Substance Abuse Coordinator on weekly basis.

7. Conducts weekly reviews of reports and prioritizes high risks patients, communicates to center clinical team and social work navigator, and links patient to care in a timely fashion.

8. Conducts and completes assessment for patient(s) and care plans as indicated.

9. Assists in the development, implementation, and evaluation of the care treatment plan.

10. Documents patient(s) encounters in appropriate data systems as required.

11. Participates in maintaining standard of nursing practice at CHN.

12. Maintains nursing competency through participation in continuing professional education and other appropriate learning experiences.

13. Work as part of an interdisciplinary care team.

14. Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.

15. Performs other duties, as assigned.

CERTIFICATE/LICENSE REQUIRED:

* Currently unrestricted Registered Nurse license/BSN in New York State.
* Current CPR certificate.
* 3+ years of clinical experience in a hospital, acute care, home health/hospice, long term care, direct care or care management, or outpatient facility.
* Experience working with individuals with behavioral health diagnoses and substance abuse preferred.
* Ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others.
* Proficient in use of Eclinical works preferred.
