(Externalcustomers include our patients, families, referral sources, vendors, the community, etc. Internalcustomers are the people within the agency with whom you work.) Displays a high degree of courtesy, tact, andknowledge of services provided by the agency in all contact with staff, patients, and visitors. 2. Assists office with achieving positive patient outcomes by: (a) responding timely to all calls to the agency duringspecified times, (b) obtains patients information on any new referrals of 3HC and notifies appropriate office,facility; (c) enters new referral information into McKesson timely and accurately, (d) conveys appropriateinformation to on-call nurse, charge nurse for each office, facility as needed, (e) consults with Administrator on-callas needed for problems, (f) notifies clinical office, facility of pertinent information by 8:00 a.m. next business day orsooner if necessary, (g) documents interactions in patient record using nursing notes, IT note, or ITC note within 24hours. 3.  Referral management training with intake staff within 30 days of hire. Training to include; (a) overall referralprocess, (b) data entry, and (c) coordination with appropriate office, facility. 4. Triages all calls to resolve issues by phone if appropriate. Coordinates prn visits by on call nurses for issues unableto be resolved by phone. 5.  Assist with orientation for new triage staff as needed.6.  Maintains regular communication with supervisor. Communicates problems of the patient(s) on a timely basisfollowing chain of command. Communicates information using current process and technology available to 3HC. 7.  Stays abreast of overall patient care and improves nursing care skills: (a) attends agency mandatory in-servicesand satisfies CEU requirements of the State Board of Nursing; and (b) strives to improve nursing care throughcontinuing education, active participation in professional and related organizations and individual research andreadings.  Demonstrates an interest in personal and professional growth: (a) attends and participates in mandatoryin-services 100% of the time; (b) attends and participates in non-mandatory in-services and other educationalopportunities as offered; (c) attends ITC/IDG meetings upon requests; and (d) does individual reading andresearch. 8. Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and theproper and safe use of supplies and equipment. #CB Job Qualifications: QUALIFICATIONS: • Registered Nurse currently licensed to practice nursing in the state of North Carolina. • Minimum of one-year experience as a professional nurse in an acute care setting. • Assessment skills proven by previous experience. • Minimum of one-year of home health and hospice experience required. • Knowledge of Medicare/Medicaid rules and regulations preferred.PandoLogic.
